TENNESSEE INSTRUCTIONS

Following are the 2 pages for you to complete for your motor club license. pay close attention to Question #4. There can be no gaps in your employment history and you must list a minimum of 5 years of history. Please include the From - To date on each employment.

You must have a TN insurance producer's license on question 6. If you do not, you must apply for it first.

Do not forget to have your application notarized on page 2.
